What is Home Health? In association with Avera Queen of Peace Home Health, Avera Weskota Home Health provides professional, health care services in your home. Avera Weskota Home Health is Medicare/Medicaid certified and is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Home Health provides many health care services to patients and their families in familiar and comforting home surroundings. What Skilled Services are Available? The following skilled Home Health services are available to patients who are homebound, have an intermittent medical need, and are certified by their physician, for skilled care. Nursing Registered Nurses from Avera Weskota Home Health are experienced and trained in current medical and nursing techniques. Some of the services they provide include the following:
Physical Therapy The primary goals of Physical Therapists from the Avera Weskota Physical Therapy Department are to improve functional ability and to promote independence. Physical Therapists evaluate and provide care to individuals who are experiencing decreased strength, problems keeping their balance, and various conditions that cause pain. Physical Therapists provide exercise, balance enhancing activities, and instruction on devices to assist with walking and transferring. Various pain management modalities are utilized. Speech Therapy Speech-language pathologists, contracted through community agencies, help with any speech, language, or swallowing disorder the patient may have as a result of illness or injury. Occupational Therapy Occupational Therapists from the Avera Queen of Peace Rehabilitation Services Department assist individuals of all ages in regaining the independence lost through injury or illness. Functional activities and/or adaptive equipment is used to maximize the individual's performance of day-to-day self-care tasks. Medical Social Worker Medical Social Workers from Avera Queen of Peace are available to assist individuals with personal, family, work, financial, and other problems that may occur as a results of an illness, disability, or injury. Home Health Aide Personal care services are provided by Home Health aides under the direction of a Home Health Registered Nurse. Some of the services they provide include:
These services are only available in conjunction with the nurse, physical, or speech therapist. DSS Services Avera Weskota Home Health has contracted with the South Dakota Department of Social Services (DSS) to provide nursing and home health aide services. Services, based on a patient's income, must be approved through a DSS case worker. Private Services If a patient does not quality for skilled or DSS paid services, private pay nursing and home health aide services are available at an hourly rate. Referral to Home Health Referral to Avera Weskota Home Health is accepted from any source. However, the patient must be under the care of a physician who will write an order for home health. The Avera Weskota program provides services within a 30-mile radius of Wessington Springs, South Dakota. Those outside this area will be referred to an appropriate program in their area. Hours and Location Home Health services are available 24 hours a day, seven days a week. Regular office hours are from 8:00 a.m. to 4:30 p.m., Monday through Friday.
